At CES Holding DBA Aquafinity we recognize our ultimate success depends on our talented and dedicated workforce. We understand the contribution each and, every Employee makes to our accomplishments and so our goal is to provide a comprehensive program of competitive benefits to attract and retain the best Employees available. Through our benefits programs, we strive to support the needs of our Employees and their dependents by providing a benefit package that is easy to understand, easy to access and, affordable for all of our Employees. This brochure will help you choose the type of plan and level of coverage that is right for you.

MEDICAL COVERAGE

Three (3) medical plans through Cigna & one (1) Kaiser HMO (Available for California residing employees only). You pay your share of the premium through pre-tax payroll deductions.

DENTAL COVERAGE

Dual PPO plan option offering offered through Cigna. You pay your share of the premium through pre-tax payroll deductions.

VISION COVERAGE

One plan through EyeMed. You pay your share of the premium through pre-tax payroll deductions.

COMPANY PAID BASIC LIFE AND AD&D

Company offers benefit eligible employees a basic life and accidental death and dismemberment (AD&D) benefit provided by New York Life.

COMPANY PAID DISABILITY PLANS

Company offers benefit eligible employees short-term and long-term disability insurance provided by New York Life. Please note, short term disability plans is company paid benefit available for employees residing in non-statutory states (CA, HI, NY, NJ, RI).

VOLUNTARY WORKSITE PLANS

Voluntary Accident, Critical Illness and Hospital Insurance plans are provided by Cigna Supplemental Health Solutions. These plans help with your out-of-pocket expenses. Most of the plans allow you to elect coverage for yourself and your family. Some of the plans have a wellness benefit. You pay the premium through payroll deductions.

One plan through Allstate. You pay the premium through payroll deductions.

PET THEFT

Two plan options available through Pet Benefit Solutions.
